Diabetes On Track

Diabetes On Track is an initiative designed to improve diabetes care and prevention in rural Nebraska communities. The effort is made possible thanks to funding from The Diabetes Care Foundation of Nebraska, which pledged $5 million to the University of Nebraska Foundation.

An initial, two-year pilot program includes two communities in Nebraska – Hastings and Wayne – and focuses on individuals at risk for prediabetes, those diagnosed with prediabetes and adults living with type 2 diabetes.

The University of Nebraska Medical Center (UNMC) is working with local health departments, health care, and other community-based organizations in both communities to help lower the rate of uncontrolled diabetes cases and to reduce the rate of diabetes progression among those with prediabetes.

The name reflects the goal: to create communities that help people with diabetes get on a healthy track through food, physical activity, and education. And ensure those at risk for diabetes or those with prediabetes stay on track to make healthful lifestyle choices and prevent diabetes.

Partners

UNMC is working closely with local health departments, health care, and other community-based organizations in both communities to address adults who may be at risk for prediabetes, those living with prediabetes, and individuals living with type 2 diabetes.
